Monitoring the tight balance of brain blood flow, oxygen delivery and brain tissue metabolic rate is a major aim in patient diagnosis and care. A patient’s health is in great danger when there is a prolonged lack of oxygen delivery to meet the metabolic demand of the tissue; for example in traumatic brain injury (TBI) where derangements of cerebral oxygen delivery and utilisation often occur, rendering the brain susceptible to secondary, or additional, injury processes. Since 2004 in collaboration with the National Hospital for Neurology and Neurosurgery in London I have performed studies in over 40 volunteers and 20 brain injured patients to explore the applicability of optical techniques to monitor brain tissue metabolism non-invasively. During this talk I will cover the instrumentation and theory behind the non-invasive in-vivo optical measurement of brain tissue energy metabolism. I will show results from the healthy and injured human brain and I will discuss the future of these measurements.
